Required Documents

• Home Care Aide (HCA) Registration Number
• TB Test or TB Risk Assessment (completed within the last 2 years)
• Social Security Card
• Valid Driver’s License or State-Issued Photo ID
• Proof of Work Authorization (if applicable)
• A recent passport-style or headshot photo for your employee ID badge.
• Auto insurance (if you drive your own car).
